 A request by a patient or a patient’s legally authorized representative for the reproduction of patient treatment and billing records, which shall be no more than $1 per page or $50 for the entire record, whichever is less, and regardless of whether such record is stored electronically, on microfilm or microfiche, or on paper…

In otherwords, it is not free, and your doctor has its own rates in giving medical notes or certificates in New Jersey.

Unfortunately, doctors’ notes are not usually covered by your insurance, but you can inquire to your coverage provider about it.
