Medical Records Request
How patients and legally authorized representatives can request, receive, or seek correction of medical records.
Who may request records
A patient, parent or guardian when legally authorized, personal representative, or another person authorized by the patient may request records. Kinder verifies identity and legal authority before release.
How to begin
Call 732-917-4454 to obtain the current secure request method or written authorization form. Written requests may also be directed to the Privacy Officer at 65 Route 1 South, Metuchen, NJ 08840. Do not send sensitive medical information through an unsecured general website contact form.
What to include
- Patient’s name and date of birth.
- Records or date range requested.
- Preferred delivery method and destination.
- Requestor’s contact information and relationship to the patient.
- Signature and any documentation establishing authority.
Timing and fees
Kinder generally acts on HIPAA access requests within 30 days, subject to lawful extensions or a shorter period required by New Jersey law. Any fee is limited to the amount permitted by HIPAA and New Jersey law. Records needed for ongoing care are not withheld because of an unpaid treatment balance.
Custody and minor records
Access may depend on custody documents, a valid court order, the minor’s authority to consent to a particular service, and laws protecting the related information.
Questions or corrections
